Babysitter handoff sheet

Complete this with the sitter before each visit. It brings the address, contacts and household rules onto one page, so nobody has to search through texts during a busy evening. Type into the fields or print it blank and write by hand.

Privacy: This sheet runs in your browser. The form does not send or save what you enter. Print or save a local PDF, then close the page when you are done. Share only the information a caregiver needs. For medical care, follow your child's professional care plan; call 911 for an emergency.

How to use the checklist
Visit and contacts
Children and routine
Home and pets
Family customs and communication

For an emergency, call 911 first. Show the sitter where this sheet and the child's professional care plan are kept. Confirm that the backup adult knows they may receive a call.

Walk through the sheet together

Do not rely on a form alone. Introduce the children, demonstrate the gate and door locks, point out the first aid supplies, and ask the sitter to repeat the address and emergency contact plan. A trusted referral still needs a clear handoff for this particular household. Revise the sheet when routines, phone numbers or permissions change.

For more detailed preparation, read the emergency contact guide, pet-care handoff, and Shabbat and holiday guide.
